You must:

• Include all history spanning the last 5 years.
• Include periods of unemployment.
• Include time spent as a student.
• Include time spent in the military.
  

Filling out the form:

• Your most current history must be listed on the top.
  • The ‘To (month/year)’  must reflect the current month and year.
• Overlapping activity is acceptable.
• Gaps in history is not acceptable.
