- Personal Email*: The personal or "home" email address for this licensee (required).
- Business Email*: The business or "professional" email address for this licensee (required).
- Business Entity Name: The associated BE name for this licensee.
- Business Web Address: The website address associated with this licensee's business.
Certain fields, as indicated above, are required. You can return to this screen later to complete your work, but all required fields must be completed before you will be allowed to submit your license application.
