Owners, Partners, Officers, and Directors (Business Entity Only)
This screen is used to indicate the Owners, Partners, Officers, and/or Directors (OPOD) associated with a Business Entity license application:
OPOD entities can include both individual and corporate (firm) entities, and can also include both entities with actual ownership, as well as entities that do not have ownership, but still possess voting interests in the associate business entity.
The "Add More" button can be used to provide additional entities. You may add both individual and firm entities to this page.
Please Note: If you provide entities with ownership (i.e. if Owner is set to "Yes" for one or more entities on this screen), the total sum of their ownership must equal exactly 100%.
For each entity you wish to add, the information required is dependent on whether the entity is an individual or a firm.
Individual Entities
An individual entity is an individual person with ownership and/or voting interest in this business entity. For each individual entity, the following information is required:
- Entity Type: Designate if this entity is an individual or a firm.
- Last Name: The last name of this individual.
- First Name: The first name of this individual.
- Title: The title held by this individual.
- SSN: The individual's Social Security Number.
- Date of Birth: The date of birth of this individual.
- Owner: This drop-down indicates if this individual is an owner ("Yes"), or not ("No"). A non-owner indicates someone who holds no ownership, but still holds voting rights.
- % of Ownership Interest: The ownership percentage of this individual. If Owner is set to "Yes", enter the percent of ownership this individual has. This field is not used if Owner is set to "No".
Firm Entities
Firm entities are firms, companies, or other non-individual owners. For each firm entity, the following information is required:
- Entity Name: The legal name of the firm or corporation.
- Title: The formal title of this firm.
- FEIN: The firm or corporation's Federal Employer Identification Number.
- Owner: This drop-down indicates if this firm is an owner ("Yes"), or not ("No"). A non-owner indicates a firm that holds no ownership, but still holds voting rights.
- % of Ownership Interest: The ownership percentage of this firm or corporation. If Owner is set to "Yes", enter the percent of ownership this individual has. This field is not used if Owner is set to "No".
