The following biographic data fields are contained on this screen:
- Business Entity Type: The entity type for this business, such as Partnership or LLC. (required)
- Business Entity Name: The name of this business entity. (required)
- FEIN: The Federal Employment Identification Number assigned to this business entity. (required)
- Incorporation/Formation Date: The date, in MM/DD/YYYY format, that this business entity was formed or incorporated. (required)
- Country of Domicile: The country of domicile for this business entity. (required)
- Financial Institution/Bank Affiliation: Designates whether this business entity is affiliated with a bank or financial institution. (required)
- FINRA Individual Central Registration Depository (CRD): The business entity's FINRA CRD number, if applicable.
