This screen is used to review and complete the background questions for the provided licensee:
Please read and consider each question carefully before answering. Some questions may have additional follow-up questions depending upon how the parent or previous questions are answered. Such questions may otherwise be grayed out based on your previous answers - such questions do not require an answer.
Depending on your answers, you may need to attach relevant documents using the Attachment Warehouse. If these documents are already in AW, you will need to verify that they are linked/associated with this application.
You can return to this screen later to complete your work, but all questions must be completed before you will be allowed to submit your license application.
