This screen is used to select the desired payment method for this application:
Users have the option of selecting to use either a Credit Card, Paypal, or an Electronic Check. Selecting an option will display the relevant billing detail fields for that payment type.
Fields marked with a red asterisk are required. When you have completed this form, click the Next button to go to the next screen.
