| 7 | License Number Sign-In: Allows you to sign in using a license number and associated state |

National Producer Number Sign-In: Allows you to sign in using a last name and associated NPN |

Social Security Number Sign-In: Allows you to sign in using a last name and associated SSN |
| 10 | Clear Input: clicking this will clear any user-entered information from any of the fields on this screen |
